According to classical texts, the pathomechanism of Sang Piao Xiao San mainly involves deficiency of both the Heart and Kidney and disharmony of water and fire. The Kidney governs water and stores essence; if the Kidney is deficient and fails to contain, the Bladder loses control, leading to symptoms such as frequent urination or enuresis. If Kidney deficiency causes failure of containment, the essence gate cannot be secured, resulting in spermatorrhea, nocturnal emission, or urine resembling rice water.

Furthermore, insufficient Kidney essence can weaken Kidney Qi, preventing it from reaching the Heart. This leads to Heart Qi deficiency, leaving the Shen (spirit) insufficiently nourished, which may manifest as forgetfulness and mental fog.
